Terms and Conditions

I have enrolled my pet into Prairie Animal Hospital’s (PAH) Wellness Membership (minimum 12-month commitment).

I understand that PAH will charge the account a non-refundable membership fee of $63 plus first month’s payment or you may elect to pay the membership in full today or at any point in the membership. A monthly payment will be automatically debited every 30 days until the end of the one year enrollment.

The Wellness Membership will automatically re-enroll at the end of each annual agreement for a total of 3 years unless you notify us at least 30 days before the end of the membership.

Financial Agreement

I understand that I am responsible for notifying PAH of any changes to my account/payment information that may interfere with payment processing BEFORE payments are due. Declined payments will result in a finance charge up to $10.00, in addition to the original payment amount due. Additionally, a $10.00 late fee will be assessed on payments that are not made within the first 7 days after the payment due date. If payment for a failed transaction is not made within a 10-day period of notification, PAH may revoke my membership and rescind all discounts given. I agree to pay full price for the services given up to the date of the declined payment.

This membership is NON-TRANSFERABLE to other clients or patients.

PAH Wellness Membership is NOT pet insurance. This membership does not cover services provided by other hospitals such as emergency clinics or specialists. The membership may be modified by PAH with or without notice.

I agree to make my payments on the date agreed between PAH and myself. I further understand that any delinquent balances may be turned over to a collection agency for further collection efforts. I also understand that any fees occurring from the use of a collection agency, attorney, or any other outside party to assist in the collection of any delinquent balances are my sole responsibility. These fees will be added to my existing delinquent balance. This includes, but is not limited to: collection fees, attorney fees, court costs, etc.

Cancellations: You may cancel your membership at any time. However, you must pay the full price of all services rendered minus what you have already paid in for the membership. Cancellation of the membership may be completed once you pay those fees or you may choose to pay out the remainder of the membership if the cost is less than the non-discounted fees. If you no longer have your pet you will still be responsible for the payment of the membership.

Memberships are the responsibility of the person who signed the financial agreement.

Extenuating Circumstances: In the event your pet passes away, you are still responsible for the remaining payments of your Wellness Plan. However, you do have the option to enroll in a plan for another pet without an enrollment fee.
